Aged and Community Care Manager
We have an exciting opportunity for a dedicated Aged and Community Care Manager to lead our Aged Care, CHSP, and Elder Support Team at YHAC!
We are seeking a highly skilled, motivated, and experienced Aged Care Manager to oversee the efficient and effective management and administration of all aspects within our Aged and Community Care Teams. You will be responsible for managing staff, optimising resources, and ensuring the highest standards of patient/resident care and satisfaction. This role requires a strong focus on working across the various Aged Care programs to ensure the facility and community programs operate in accordance with the Aged Care Quality Standards and Commonwealth Government Funding agreements.

Key Selection Criteria:
- Tertiary qualification in Business, Management, Nursing or Health
- A passion improving service and client outcomes in Aged Care
- Previous experience as an Aged Care Manager or similar role, with a proven track record of successfully managing multiple programs or areas simultaneously
- In-depth knowledge and understanding of the Aged Care Quality Standards and of the industry itself, in order to make informed decisions and to ensure the facility meets accreditation standards
- Demonstrated ability to manage budgets, allocate resources efficiently
- Proficiency in utilising electronic health records (EHR) and other healthcare management software
- Excellent communication and interpersonal skills with ability to build relationships with patients, families, and healthcare professionals
- Excellent organisational and time management skills with the ability to manage competing tasks and priorities
- Current National Police Check, DHS WWCC, and DHS Aged Care Screening
- Current valid Driver Licence (minimum of Class “C” or equivalent)
- Evidence of Current Flu Vaccination, and Covid-19 1st, 2nd & Booster Vaccinations
Key Responsibilities:
- Lead and manage the Aged and Community Care Teams to ensure YHACs vision, mission, and values are met
- Review current operational policies, procedures, and protocols and implement changes/improvements, in conjunction with relevant staff and senior management, to ensure efficient functionality of all Aged and Community Care Teams
- Manage Aged and Community Care resources, including staffing, scheduling, budgeting, and inventory efficiently
- Oversee and assess the Aged and Community Care programs performance and delivery, identifying areas for improvement and implementing appropriate strategies to ensure YHAC is meeting the needs of the Aboriginal community
- Foster a positive work environment that encourages teamwork, open communication, and professional growth
- Manage staff performance and provide regular analysis of staff skill levels to determine ongoing suitability for their roles, ensuring staff are best placed within the organisation appropriately to achieve required accreditation outcomes
- Ensure staff are building and maintaining strong relationships with residents/elders and their families, addressing their concerns and feedback in a timely and satisfactory manner
- Collaborate with other healthcare professionals and external stakeholders to enhance patient care and service delivery
- Ensure compliance with the Aged and Community Care requirements and guidelines, accreditation standards and agreements/contracts
