The Organisation

Rural Workforce Agency Northern Territory (RWA NT) are partnering with Wurli Wurlinjang Health Service (Wurli), to find a Credentialed Diabetes Educator to join their well-established passionate team. Wurli is an Aboriginal Community Controlled Organisation delivering primary and clinical health care services to the Aboriginal people in Katherine and surrounding communities. Wurli prides itself on delivering a range of health services to the community in a style and manner that makes clients feel welcome and comfortable. Wurli operates a range of clinics and provides a range of primary health care and other services which include main clinic (acute and general care), Gudbinji Chronic Disease Clinic, Binjari Health Centre, Women and Children's Health and Men's Health. 

The Role

The successful Diabetes Educator will work closely with staff from all programs to provide a comprehensive general practice service for the people with diabetes and their families. The Diabetes educator is required to be flexible, multi skilled and able to work with a diverse range of individuals, groups and community agencies. The role incorporates such activities such as advanced clinical care, self-management support, running education and support groups and providing professional development to other clinicians.

Key Responsibilities

To provide expert delivery of safe and effective evidence based clinical care and effective education to people with Diabetes Type 1, Type 2, GDM, IGT, which include but not limited to:

• Communicate with all clinical staff to ensure clients are referred to Diabetes services appropriately and are provided service in a timely manner
• Conduct consultations at the main clinic 2 days per week
• Conduct consultations at the women's clinic 1 day per month
• Attend camp run clinics Tuesday morning
• Meet with program coordinators quarterly to develop needs based education and client centred care objectives
• Initiate treatment using CARPA standards as the clinical guidelines
• Assess and provide Diabetes care and determine eligibility for service and make referrals where appropriate
• Develop an appropriate treatment plan and goals in consultation with the client to achieve success
• Assist and educate Aboriginal and Torres Strait Islander people to understand Diabetes and how to manage it
• Advise clients on the importance of following a care plan, which may include structured support for self-management and assistance with care plan preparation
• Maximise community and individual self-reliance, self-care and participation through innovation with client and community driven focus
• Contribute to closing the gap in life expectancy by improved access to Diabetes education and prevention pathways that are culturally appropriate
• Collaborate with other agencies and sectors to support an integrated referral and advocacy system
• Liaise with the health promotion team to provide culturally appropriate learning tools for clients
• Actively encourage clients, family members and the community to engage in practices conducive to optimising health
• Work effectively within a multi-disciplinary team to promote productive relationships
• Perform accurate and timely entry of client data into Communicare (the health data management system) on a daily basis
• Ensure personal safety of yourself and others are maintained 
• Promote and embody Wurli's vision and values and act as a role model for staff and clients

Essential Criteria

To be considered for this opportunity, you must meet the below essential criteria:

Only applicants with full working rights within Australia will be considered (Australian Citizen or Permanent Residents)

• Current Resume with two contactable references
• Current Registration with AHPRA as a Registered Nurse or Aboriginal Health Practitioner
• Credentialed Diabetes Educator status
• Five years clinical practice experience, including the provision of chronic disease care
• Be able to collaborate and cooperate with others to achieve common goals
• Have excellent communication, interpersonal and analytical skills
• Be a problem solver with the ability to effectively address a range of issues as they occur
• Ability to prioritise, organise and handle numerous competing tasks
• Be passionate about improving the well being outcomes for Indigenous
• Current National Police Clearance and Working with Children (Ochre Card)
• Current Northern Territory Driver's License

Desired Criteria:

• Diploma of Aboriginal and Torres Strait Islander primary health care practice or Master of Public Health
• Previous experience in providing chronic disease/diabetes care
• Experience in the use of Retinopathy Cameras
• Experience in working with a community controlled Aboriginal health service
• Broad understanding of the health system in the Northern Territory

The Location

The Katherine Region

The town of Katherine is the thriving main town within this sub-tropical region, situated a three hour drive south of Darwin. The main town is situated on the banks of the Katherine River and has a population of around 10,000 people while the larger Katherine region has a population of just over 24,000. Katherine is well equipped as a regional town with a shopping centre that features a supermarket and many specialty shops. Katherine is on the edge of Kakadu National park which is filled with waterfalls and bush walks. Kayaking through the picturesque Katherine Gorge is a popular weekend activity.
