NT PHN are partnering with Sunrise Health Service Aboriginal Corporation, to find a Credentialed Diabetes Educator, to work in and provide excellent healthcare to their remote communities. Sunrise Health Service Aboriginal Corporation is an independent community controlled health service that provides comprehensive primary health care services to nine remote health centres and to the 4000 people living in the Katherine East region. Health centres are located at Barunga, Manyallaluk, Wugularr, Bulman, Mataranka, Jilkminggan, Minyerri, Ngukurr and Urapunga. The Role:
The successful Credentialed Diabetes Educator provides services to all Sunrise clinics and communities providing individual and group services to diabetic patients and families.
Being part of a passionate team of preventative and chronic disease health professionals, the role will be based in the wider Population Health team that includes a range of other clinicians and programs.
The successful candidate along with the Population Team members, will travel regularly to the nine remote clinics, and some overnight stays will be required.
Key Responsibilities:
- Provide high quality and individualised diabetes management and care for patients and their families suffering from, or at risk of diabetes and associated conditions
- Work collaboratively with all health centre and specialist staff to ensure appropriate planning and implementation of care for patients receiving care for their diabetes and associated conditions
- Work as a member of the Chronic Disease team and communicate effectively within the multidisciplinary team and promote a team approach to decision making and care provided
- Provide clinical leadership to other health professionals including Remote Area Nurses, Aboriginal Health Practitioners, and medical and allied health professional staff in the area of diabetes management
- Initiate and promote patient referral to other health professionals
- Manage Medical Benefits Scheme claims for care provided as required and applicable
- Implement and promote evidence based standards and policies in accordance with clinical and practice guidelines and Territory and Federal legislative and regulatory requirements
- Develop and utilise sound and sustainable working relationships and networks with other organisations at both the community, inter-service and professional levels
- Record all work on patient information system and contribute to regular reports to funding bodies
- Provide collaboration in the coordination and implementation of quality improvement activities
Essential Criteria:
To be considered for this opportunity, you must meet the below essential criteria:

- Current Resume with 2 contactable referees
- AHPRA registration as a Registered Nurse or Nurse Practitioner
- Credentialing as a Diabetes Educator
- Extensive professional experience in diabetes and chronic disease management
- Advanced interpersonal and communication skills both written and verbal
- Have well-developed analytical and problem solving skills
- Demonstrated understanding of the principals of Aboriginal community controlled health services
- Ability to practice in a sensitive and culturally safe manner in a remote Aboriginal Community
- Demonstrated knowledge of comprehensive Primary Health Care Principles, including social determinants
- Knowledge, understanding and experience in quality improvement, practical application in meetings client's needs and its relationship to strategic development
- Proven experience, knowledge and understanding of research findings to support evidence based best practice
- Current Criminal History check
- Current Working with Children (Ochre Card)
- Manual Driver's License with 4WD drive skills

The successful candidate must be willing to travel in light aircraft or four wheel drive and stay overnight in the Sunrise Health Service remote communities, as required.

The Location: Katherine Region
The town of Katherine is the thriving main town within this sub-tropical region, situated a three hour drive south of Darwin. The main town is situated on the banks of the Katherine River and has a population of around 10,000 people while the larger
Katherine region has a population of just over 24,000. Katherine is well equipped as a regional town with a shopping centre that features a supermarket and many specialty shops.
Katherine is on the edge of Kakadu National park which is filled with waterfalls and bush walks. Kayaking through the picturesque Katherine Gorge is a popular weekend activity for at least 40,000 years. The Yolngu people are famous for their bark paintings and traditional Aboriginal culture.
