This client is an award winning Not-for-Profit that specialises in providing housing solutions in Sydney and regional Australia, to individuals and families on low to moderate incomes who are unable to access housing.

I am seeking an experienced Case Manager, with good working knowledge in out-reach / homelessness / Community-Social housing. You will be an integral part of the Together Home Program team, based at the Parramatta office. You will be supporting approximately 15 clients, with a focus on the Greater Western Sydney region.

Assignment dates 05th June - 25th July 2023. Generally, your hours will be 9am-5pm (they may need to flex a little, depending on the client's needs), Monday to Friday only, as there is no on-call or weekend work. Your hourly rate is $56.15 + super, paid weekly. You will be provided a fleet car, to take home (including weekends), as you will be predominately out and about with clients and in the community.

The Together Home program specifically works in homelessness services and people in temporary accommodation. As the Case Manager your role is to provide, strength based, client centred, trauma informed wrap around case management. You will assess and coordinate client referrals, complete interviews, and client intake. Develop and support case plans and referrals in collaboration with clients, including case review and exit planning. You will also link clients with training and employment opportunities and other support services including local health districts, helping them to build good working relationships with the services. The goal of the Together Home program is to help improve client's overall wellbeing, independence, and social inclusion.

What you need to be successful:

  • Ideally you will have knowledge of the social and community housing sector
  • Strong case management experience and exposure to working with clients with a variety of complex needs and diverse backgrounds
  • Outstanding communication skills, active and empathetic listening skills and strong written skills
  • Knowledge and understanding of issues that contribute to homelessness and rough sleeping
  • A willingness to travel and work across a number of locations to support your clients
  • Ability to work independently and as part of a team
  • Ability to mediate under pressure to resolve differences using a strength's-based approach.
  • Current unrestricted, Australian drivers' licence

Checks & Qualifications:

  • Tertiary qualifications in Social Work, Psychology, Welfare, or related areas
  • Australian Driver's License
  • WWCC or willingness to obtain one
  • NDIS Check or willingness to obtain one
  • National Police check
