We are seeking a Family Violence Practitioner to step into our new Entry Point team! The salary for this role is a SHADS Level 5 (between $88,761.92 to $92,773.20 plus superannuation).

The new Entry Point team has been created to receive case management referrals from the Orange Door, self-referrals to GenWest, or secondary consultations from other community services.

The Entry Point team will provide case management services with a brief intervention focus (less than 8-10 hours). This may include providing interim case management services to someone who requires longer term support whilst waiting to be allocated the Case Management Team

The Entry Point will also coordinate Information Sharing requests, and subpoenas responses while working with the Crisis Response Team, Case Management team and RAMP team to ensure effective internal referral that is focused on clients’ risk profile.

The service provides trauma informed responses to victim survivors through strengths-based case management including family violence information, safe planning review, referral to other services, and supporting the victim survivor to develop a pathway to recovery.

The key responsibilities of this role include:

• Utilise the MARAM framework to conduct comprehensive risk assessments, needs assessment, and safety planning for victim survivors
• Working collaboratively with external services utilising relevant information sharing legislation, to ensure quality of risk assessments and risk management practice
• Provide brief case management including the capacity to hold a case load
• Determine victim survivor eligibility criteria for the service, including exploring referral pathways, empowering victim survivors by providing them with information and advice in order for them to make informed choices.
• Engage in learning activities to support ongoing professional development
• Be part of team who are rostered to provide Intake and Triage to clients who are requiring a service, via phone or face-to- face.
