Due to continued growth and transformation, Windermere is seeking a Specialist Support Coordinator to join a highly skilled and dedicated team who are committed to providing high quality services and go above and beyond for our consumers to coordinate and implement the support they need.

About the role

As a Specialist Support Coordinator, you will play a pivotal role in supporting and empowering families and NDIS consumers living with highly complex needs to navigate the NDIS, identify goals and implement the NDIS plan, as well as strengthen their capacity and build their resilience to create lifelong networks and connections in order to access support and become – and remain – independent. This consumer facing role is a permanent ongoing full-time position.

Windermere promotes a hybrid model of working including working remotely from home, outreach (delivering services in the consumers natural environment at home) or within our head office in Narre Warren.

About You

You will hold tertiary qualifications in Social Work, Psychology or Allied Health and hold a current registration with or be eligible to register with your professional association. Furthermore, you will have relevant knowledge of the NDIS principles and demonstrated experience in Support Coordination, as well as customer-centric risk assessment practices in identifying and managing complex needs.
