About the role

The NDIS Occupational Therapist is responsible for quality community-based services to NDIS participants. This may include home or centre-based assessment, living skills training, home modification and equipment prescription following a prescribed NDIS Support Plan. Although the current support plan is the primary emphasis of the role it remains within the scope of the role to use chronic disease management and health literacy principles to refer the participant to relevant and appropriate health services.

Your daily duties will vary but will include:

Direct Care Services

  • Provide high-quality Occupational Therapy services to cohealth NDIS clients in accordance with billable hours stated on the Support Plan
  • Liaise with a range of stakeholders to ensure comprehensive and effective client management in accordance with client Support Plan goals and timelines
  • Liaise with cohealth Support Coordination team, participate in coordinated care activities and liaise with Local Area Coordinator to advocate for clients' current plan and future NDIS goals
  • Refer, consult and liaise with staff within the organisation and other local agencies to ensure appropriate on-referral and management of health-related issues
  • Work within a team to evaluate and improve key NDIS workflows such as referral screening, client acceptance and billing on an ongoing basis to optimise service delivery and improve the participant experience
  • Incorporate health education into NDIS contacts using health literacy principles
  • Perform other duties consistent with the aims and objectives of cohealth as directed by the program facilitator and practice excellence coach

Health Education & Health Promotion

  • Collaborate with and advise other disciplines around the creation of NDIS billable opportunities
  • Act as an advocate for individual clients/groups of clients as appropriate
  • Establish and maintain effective working relationships with stakeholders (NDIA NDIS, SWEP, LAC, OOH) and participate in team and organisational activities

Quality Improvement

  • Ensure the safety and maintenance of Occupational Therapy equipment and supplies
  • Adhere to relevant policies including infection control policy
  • Participate in supervision as per allied health and nursing supervision framework
  • Participate in quality assurance and audit activities to evaluate safe practice
  • Develop and lead organisational and discipline-specific Quality Improvement activities
  • Complies with the clinical governance framework supporting this position- including formal support from Program Facilitator and Occupational Therapy Practice Excellence Coach

Administrative responsibilities

  • Maintain and complete client files and statistical/program records in keeping with the organisation's policy and funding requirements
  • Have appropriate knowledge or willingness to learn of NDIS funding guidelines and be accountable for meeting those requirements
  • Participate in the recruitment and selection of staff as appropriate
  • Monitor and order educational materials, literature and Occupational Therapy equipment

Team & Policy Development

  • Participate in team meetings and Occupational Therapy team meetings
  • Participate in the planning, development and evaluation of the Support Coordination Team
  • Professional conduct in the execution of all duties by regulating their own behaviour and understanding organisational structure and culture
  • Establish and maintain effective working relationships with stakeholders and regularly participate in team and organisational activities

About you:

  • Excellence in the delivery of community-based, adult Occupational Therapy services
  • Demonstrate ability to work effectively within a multidisciplinary team (face to face or virtually)
  • Demonstrated ability to work effectively with marginalised clients and those who have complex needs, including CALD groups
  • Current registration with Australian Health Professionals Regulation Agency as an Occupational Therapist, or eligible to register
  • Commitment to professional development and quality improvement
  • Highly developed communication and interpersonal skills
  • Demonstrated commitment to community participation and ongoing engagement with consumers
  • Well-developed computer skills
  • Proven ability to manage crisis situations in order to maximise the safety of clients and self
  • Well-developed written and verbal communication skills
  • Competent IT user
