Who are we?
Wintringham is a specialist not-for-profit company providing an extensive range of aged care, housing and related support services to elderly men and women who come from a homeless background. We employ over 1000 staff and operate from multiple offices in Melbourne, Regional Victoria and Hobart. Our regional team of Carer’s, Case Managers, Outreach Workers, Care & Support Workers and Recreation staff provide holistic care and support to clients who live independently in their own homes.
Your new role
We are currently seeking a flexible and enthusiastic Housing Support & Outreach Worker to join our regional site based in Shepparton on a Part Time fixed term basis, on the 31st of March 2024 for 9 days per fortnight, however we would consider a 0.8 FTE schedule for the right candidate .
Responsibilities and duties will include:
- Provide housing support, conduct assessments, and document plans for Wintringham residents and Care Finder outreach clients facing homelessness.
- Connect clients to clinical and social services, assisting with referrals and case management.
- Maintain accurate client records and monitor service quality.
- Address social isolation, help maintain tenancies, and respond to client feedback.
- Promote a client-focused approach, adhere to policies, and ensure safety and confidentiality.
What we require from you:
- Experience in a support worker role and/or case management with specific training and/or experience in AOD, mental health and family violence or homelessness services
- Proven commitment to excellence, accountability and innovation in best practice service delivery in the fields of homelessness and housing support
- Knowledge of Commonwealth and State government policies, standards and compliance requirements
- Knowledge of and commitment to continuous quality improvement
- Commitment to the organisation and its philosophy of social justice
- Well-developed communications skills and the ability to engage with people from varying backgrounds
- An understanding of the housing and homelessness service sector, My Aged Care, NDIS and other relevant service systems

We are looking for candidates with the following:
- A qualification in Social Work, Social Sciences, Welfare or equivalent
- Outreach skills including the ability to locate and establish rapport with both socially isolated people and people with challenging behaviours
- Experience in working effectively and sensitively with a broad range of clients
- An understanding of the Housing and Homelessness Service sector, My Aged Care, NDIS and other relevant service systems
- A current drivers licence, and reliable vehicle
- Proven commitment to excellence, accountability and innovation in best practice service delivery in the fields of homelessness and housing support
- Well-developed communication skills and the ability to engage with people from varying backgrounds
- First Aid Certificate
