The role you could play

We are seeking a Case Manager for our Supported Accommodation Network Service! You will be responsible for providing assertive outreach support to men and women experiencing homelessness to rough sleepers. Additionally, as Homelessness Case Manager for the SANS program, you will ensure individual complex care for single people over the age of 18 with intensive complex needs.

What is the Supported Accommodation Network Service?

The SANS program provides person-centred and trauma-informed support ensuring access for clients to relevant services. These services include advocacy in obtaining safe and secure housing, referral in relation to legal issues, income security, health issues, drug and alcohol problems, specialised counselling, and psychiatric issues.

Duties

  • Provide clients with a case management service that addresses identified needs, goals and risks in areas such as housing, health, mental health, AOD issues, legal issues, family issues, financial issues and employment
  • Assist clients to develop a housing plan which includes pursuing appropriate, affordable long-term housing and obtaining the most suitable available interim accommodation
  • Place clients in Transitional Housing and community rooming house accommodation, on the basis of identified criteria
  • Assist clients in Transitional Housing to maintain their tenancy and resolve issues with the THM provider, to the point of obtaining appropriate long-term housing
  • Case management support to clients in accordance with TSA case management models

This is a permanent full-time position based in Flagstaff, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.

The ideal candidate will have

  • Diploma or Degree qualifications in Social Work, Psychology, or related field is required
  • Relevant experience in the social service sector including case management & assertive outreach practice
  • Ability to demonstrate knowledge of homelessness including context relative to best practice
  • Experience working with internal and external stakeholders to both maintain and create new connections in the sector
  • Disclosure of pre-existing health conditions that may impact on fulfilling position requirements
  • Proof of COVID Vaccination required
