Homelessness Melbourne Metro is committed to providing person-centred care, trauma-informed practice and a strengths-based approach to improve the health, wellbeing and housing outcomes for people experiencing homelessness.
We are seeking a Case Manager for our Supported Accommodation Network Service! You will be responsible for providing assertive outreach support to men and women experiencing homelessness to rough sleepers. Additionally, as Homelessness Case Manager for the SANS program, you will ensure individual complex care for single people over the age of 18 with intensive complex needs.
The SANS program provides person-centred and trauma-informed support ensuring access for clients to relevant services. These services include advocacy in obtaining safe and secure housing, referral in relation to legal issues, income security, health issues, drug and alcohol problems, specialised counselling, and psychiatric issues.
This is a permanent full-time position based in Flagstaff, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.
