About the role

The recently conceived Care Finders initiative is aimed at connecting senior Australians to aged care services by providing community-based specialists with extensive knowledge of the aged care system to help understand their needs and navigate the complex system to access the supports, care, and funding they require. This is where you come in. You have an opportunity to join this exciting new initiative from the start and provide person-centred services to elderly people at their point of need to enable them to exercise greater choice, control, and independence.

As a Care Connector you will:

  • Work with clients in the community to understand their needs and help them access and connect with the supports they need
  • Utilise your extensive knowledge of the aged care sector, including the My Aged Care portal, to help your client navigate the system to obtain referrals and access to services
  • Leverage your networks and relationships with key agencies, other providers, and referral services to help identify, access and coordinate the supports your clients require.
  • Follow up periodically with your clients to ensure they continue to receive the services they need and provide ongoing support where required

Skills and Experience

In addition to a passion for helping Senior Australians live their best life, you will be able to demonstrate the following:

  • Relevant qualifications and/or experience in the aged care sector
  • Local community connections with the target population or a specific sub-group within the population
  • ​A detailed understanding/ability to rapidly attain a detailed understanding of the range of aged care supports and services.
  • ​A commitment to delivering a person-centred approach that respects and responds to each person’s individual needs, preferences, values and life experiences
  • ​An ability to communicate effectively and support the needs of people with diverse backgrounds and life experience, including Aboriginal and Torres Strait Islander, Culturally and Linguistically Diverse (CALD), and LGBTQIA+ communities
  • Strong networking and relationship building skills as well as the ability to work collaboratively both within your team and between agencies or organisations.
  • A self-motivated approach to be able to identify a person’s needs and the tenacity and problem-solving skills to help them access the supports they need.

If you are a person of Aboriginal or Torres Strait Islander background your cultural competency and/or language skills will be highly regarded.

If you are a Culturally or Linguistically Diverse person, your multilingual and/or cultural competency will be highly regarded.
