We are leading the way in the sector with the use of metric tools for internal auditing within our Community Services and Supported Living services. This helps us to know when we are on the right path and when and where we need to make improvements in our services.
Purpose of role:
The role of the Community Support Worker is to actively support and promote the independence of our clients by providing quality services using a person centered approach that focuses on developing their potential to participate actively within the community and their home.
Reports to:
Service Manager / House Service Coordinator
Environment:

- Community Support Workers are employed under the Social, Community, Home Care and Disability Services Industry Modern Award 2010.
- Sunnyfield Supported Living Community Support Workers will: work in community based group homes and work a varied roster of shifts which may include weekends and overnight
- Community Support Worker's employment is with Sunnyfield, inclusive of all Sunnyfield services.
Duties of the role:
- Assist our clients to be involved in activities through active support.
- Assist and prompt our clients to maintain the home environment which will include completing and assisting with tasks if the person is unable to complete.
- Ensure that quality service is provided to meet individual's specific health needs.
- Assist clients to maintain a high level of personal hygiene, personal grooming and personal care.
- Support and assist our clients with medication as required in compliance with Sunnyfield Medication procedures.
- Encourage, support and assist our clients to develop and maintain family and community relationships.
- Become familiar with the individual wants and needs of our clients to provide the best possible service with the available resources.
- Transportation of our clients to community activities, facilities and events.
- Demonstrate adherence to all plans that are developed to provide support for health care, communication, meal time and any areas that require specific support plans.
- Assist our clients to achieve their personal goals and increase their independence through support and participation in decision making.
- Writing daily progress notes for every person you support and keeping records, general files and all relevant documentation up to date.
- To monitor and maintain clients' personal monies and belongings.
Knowledge and skills:
- Broad knowledge of services for people with disabilities.
- Knowledge of NSW Disability Services Act 1993, NSW Disability Service Standards and other relevant legislation.
- Demonstrated high level of written and verbal communication skills.
- Well developed time management skills within competing priorities.
- Demonstrated ability to work independently and as an effective team member.
- Exhibit a personal commitment to service, integrity and professional development.
- Basic MS Office skills necessary
Qualifications and experience:
- Current First Aid Certificate (desirable).
- Current NSW Class C Driver's Licence (essential).
- Certificate IV in Disability Services (or willingness to obtain).
- Current WWC Check
Criminal record checks with the NSW Police, Working with Children checks, prior employment checks and functional assessments are part of Sunnyfield's recruitment process.
