About the Role

This role has dual responsibilities including to:

• Provide clinical leadership for the SVHM Check-In staff based at Frontyard Youth Service ensuring delivery of an efficient, effective and clinically therapeutic service. The Senior care coordinator will manage day to day clinical operations of the Check-In staff, rostering, monitor and support team activity achievement, support recruitment activities, staff orientation and staff performance development, provide clinical and professional advice and supervision to the team, lead case conference and other team specific meetings, and ensure the staff operates within accepted best practice guidelines.
• This role will also manage a small caseload of clients within the Check-In program and provide brief opportunistic support to users of the drop in service.
• This position will receive clinical/professional support from through the ALERT leadership team. Operational leadership onsite will sit with Frontyard.

The classification range for this position ranges from:

• Community Health Nurse (In Charge) RN4B/CN6 ZA1 ($113,646 per annum, pro rata)
• or, Allied Health Grade 3:
  • Physiotherapist Gr 3 VB7 - VC1 ($98,129 – $112,117 per annum, pro rata)
  • Occupational Therapist Gr 3 VG3 - VG6 ($98,129 – $112,117 per annum, pro rata)
  • Social Work Gr 3 SC31 - SC34 ($98,129 – $112,117 per annum, pro rata)

Your Contribution

• Nursing or Allied Health qualification with minimum 7 years experience in relevant acute/community setting.
• Eligible for registration/membership with AHPRA
• Current Victorian Driver’s Licence
• Current or willing to undertake Working With Children Check and Police check

About Healthcare for the Homeless and Frontyard services

This position is part of the Health Care for the Homeless Program (HfH). HfH comprises several services that support clients that are experiencing or at risk of homelessness. HfH aims to integrate and better coordinate existing services within St Vincent’s Hospital and where service gaps are identified, support the development of new services to meet the needs of our clients.

Operationally the role is based at Frontyard Youth Services which is a specialist youth service located in Melbourne’s CBD. Frontyard aims to support young people aged 12 to 24 to meet their physical, emotional and social needs and to develop pathways out of homelessness. This includes but is not limited to providing 18 beds crisis accommodation service and drop in center housing multiple services working with young people across Greater Melbourne and throughout Victoria.
