The Organisation

Sydney North Health Network (SNHN) is the Northern Sydney Primary Health Network (PHN), part of a national program of primary health networks designed to increase the efficiency and effectiveness of health care for patients, particularly those at risk of poor health outcomes.

Sydney North Health Network works with a broad range of stakeholders to transform the health system in the region by building synergies, streamlining approaches, and creating momentum for population health improvement.

The Role

Reporting to the Executive Manager, Coordination & Integration, the role will lead and manage the expansion of care coordination and integration to improve access and navigation of services. You will address gaps in service provision and improve service integration. Additionally, you will be a key interface between health service providers and SNHN.

Specifically you will:

  • Develop & implement efficient and effective care coordination and integration programs
  • Lead strategies for coordination and integration across a range of services including aged care, mental health, suicide prevention, and AOD
  • Oversee a small team of professionals including project coordinators and subject matter experts
  • Support primary care providers to develop their capacity to deliver coordinated primary care services
  • Be involved in extensive stakeholder engagement with government departments, GPs and allied health professionals

This role is offered for an initial contract period until mid-2023, but the intention is that this role be ongoing, dependant on funding.

Skills Required

You will have significant experience in health service planning or delivery, health project delivery or health policy review and improvement.
You also have:

  • Experience setting and delivering agreed outcomes and overseeing a small team
  • Expert written and oral presentation skills
  • Strong interpersonal skills, including persuasion and negotiation skills, with the ability to participate in advisory bodies and influence senior stakeholders in strategic operations
  • The ability to present information effectively to a variety of audiences
