Young Adults Support Services Teams provide a diverse range of integrated programs that provide early intervention and complex case management to young people aged 16-25 at any given time. The service also administers the delivery of 90 private rental brokerage packages across the North/West regions of Melbourne per annum.
The Youth Justice Case Manager works out of the Northern Community Hub, providing support and accommodation to homeless young people aged between 17 to 25 years who are exiting custodial settings. The programs aim to assist young people in securing and maintaining affordable housing by addressing unmet needs through an outreach case management model. The role advertised is a dual role that combines both Youth Justice Community Services Support (YJCSS) and Youth Justice Homelessness Assistance (YJHA).
You are an appropriately qualified professional with experience in providing comprehensive case management, planning and support to assist clients within a homelessness setting. You have experience in maintaining constructive, empathetic, supportive and respectful working relationships particularly for an integrated service response.
