About the role

VincentCare Victoria is seeking an, energetic, and committed RN to undertake clinical assessment for HCP clients, have an oversight of Nursing services and clinical services provided by contracted partner's to HCP clients in the community and provide consultation and guidance to both the Case Management and Support Worker teams. This is performed in accordance within the scope of practice of a RN as determined by the Australian Health Practitioner Agency (AHPRA) and the Nursing and Midwifery Board of Australia.

Responsible to conduct clinical assessments and reviews, liaise with mainstream and specialist health providers, and provide secondary consultation with other program staff

Coordinate the development, implementation and ongoing review of clinical assessment and care planning including areas of specialised nursing in an outreach capacity

Review and respond promptly, effectively and appropriately to changes in client's health status

Ensure client care is delivered in accordance with the Aged Care Quality Standards and Organisational values, standards, policies and procedures and to the concept of continuous quality improvement.

What We Are Looking For:

  • Current registration with Australian Health Practitioners Regulation Agency (AHPRA) as Registered Nurse (Division 1) with no restrictions on registration to practice
  • Aged Care experience - preferably community based
  • Able to travel to Both Gippsland and Shepparton regions to conduct clinical assessments for clients in these regions
  • Understanding of accreditation process and the requirements of the Aged Care Quality Standards as they apply to in a Community setting
  • Be able to work autonomously and collaboratively within a team setting
  • Understanding of, and ability to undertake clinical assessments and develop or inform care plans
  • Ability to take necessary initiatives and make appropriate judgments regarding clinical care
  • Excellent customer service skills
  • Empathy, patience, and a strong sense social justice.
