About the Role

The Local HealthPathways Coordinator contributes to SNPHN’s vision by supporting the implementation and maintenance of the HealthPathways program’s strategies and objectives. HealthPathways is an on-line portal for providing referral and management information for General Practitioners.

Sydney North Primary Health Network (SNPHN), is one of 31 Primary Health Networks (PHNs) established by the Australian Government to increase the efficiency and effectiveness of medical services for the community. Our focus is on patients who are at risk of poor health outcomes and we work to improve the coordination of their care so they receive the right care, in the right place at the right time. Better health outcomes for patients is achieved by working together with a network of health professionals including general practitioners, practice nurses, allied health providers, the Northern Sydney Local Health District and other health services. Key Responsibilities:

  • Ensures good relations with key stakeholders involved in HealthPathways, are developed and maintained.
  • Ensure data integrity for all relevant systems is maintained;
  • Contribute to design, development and maintenance of the HealthPathways website
  • Ensure administration of the HealthPathways website and updates on pathway status completed in agreed timeframes
  • Ensure clinical content on HealthPathways is of high quality, is relevant and current
  • Ensure workstreams prioritised reflect area needs and is supported by data-based evidence
  • Ensure risks and issues are flagged in agreed timeframes to the Manager Systems Integration & Design (SID)
  • Ensure the successful implementation of the local HealthPathways program; supporting the Manager SID, NSLHD Program Manager, Clinical Editor(s), Subject Matter Experts and Workgroups as required to progress local pathway development and use.
  • Actively support the Clinical Team, ensuring internal and external communications requests, resource needs and other required tasks are met.
  • Train Clinical Team to use the HealthPathways draft website to develop local pathways and use of Dot management system.

The ideal candidate has:

Essential:

  • Tertiary health related qualifications e.g. nursing, allied health, health sciences or similar
  • Experience in project coordination or project support of multiple projects
  • Ability to liaise, collaborate and consult with a range of internal and external stakeholders including senior clinical and executive staff
  • Demonstrated understanding of primary healthcare in an Australian setting and understanding of the importance of an integrated network with acute care services
  • Experience in facilitating and/or coordinating meetings and/or forums
  • Effective planning, organisational and problem-solving skills
  • High attention to detail and ability to manage competing priorities
  • Demonstrated oral and written communication and interpersonal skills and the ability prepare complex reports and submissions
  • Demonstrated ability to persist with programs despite challenges & setbacks that may occur
  • Proficiency in MS Office and related applications

Desirable:

  • Experience in a not for profit, government funded environment
  • Demonstrated data collection and analysis skills
  • Experience with CRM databases and web-based portals

Certifications Required:

  • Evidence of formal qualifications and/or professional memberships, as applicable to the role
  • Current NSW drivers’ licence and access to a comprehensively insured motor vehicle – if a requirement of the role.

Remuneration:

A base salary commensurate with skills and experience

Special conditions:

You will require some flexibility in your work schedule to meet business needs which may require some out of hours work – evenings and weekends, for example, attendance at forums or meetings. Some intrastate and/or interstate travel may be required.

This is a full-time role for the period up to June 30, 2021.

Successful candidates will be required to consent to a National Criminal Record, Reference and potentially Working with Children Checks if a requirement of the role.
