We are designing and implementing innovative approaches and models that enable healthcare providers across all settings (acute, aged and primary care) to work more seamlessly together. Our aim is to provide more options for frail and elderly people requiring access to rapid or urgent care in the community so they do not need to attend hospital.

This is a unique opportunity being supported by the Ministry of Health through its Collaborative Commissioning initiative, which provides a framework for collaboration to achieve high value, integrated and outcome-based care. Northern Sydney is one of the first regions to progress the initiative in NSW.

The Senior Project Officer will support SNHN and NSLHD in the development and implementation of innovative initiatives and models for frail and elderly people. The Senior Project Officer will undertake a range of activities across the commissioning cycle (needs analysis, co-design, procurement, implementation and evaluation), including:

As a Senior Project Officer, it is expected that you will undertake these activities with minimal supervision and demonstrate sound judgement in managing your tasks.
