About the role

We are seeking an experienced Support Coordinator to join the SHMOW NDIS team. Reporting to the NDIS Program Manager, the Support Coordinator will provide non-clinical community-based support and build respectful and compassionate relationships with participants. You will follow the participants NDIS support plans to build resilience, confidence and independent living skills including (but not limited to) assisting participants to appointments, provision of transport, enhancements of daily living skills and developing capacity building strategies to increase skills, achieve personal goals and build independence. You will:

  • Engaging and coordinating service providers on behalf of participants and link participants to relevant providers
  • Meeting with and engaging participants to develop and refine their goals
  • Engage participants informal supports (family, carers, etc.)
  • Working with participants to help build their own capacity and independence
  • Building knowledge of local services and organisation’s customers can utilise
  • Engaging with NDIA staff and LAC’s
  • Support clients to maintain and connect with family, friends, peers, community, and other services and supports
  • Adhere to service agreement requirements of the funded package and meeting of deadlines
  • Have accountability for effective utilisation of participant’s plans for all clients allocated
  • Maintain accurate and up to date documentation in SupportAbility

Key requirements

  • A values-based approach to work aligned with our mission
  • Previous experience in a direct service delivery role within mental health, homelessness and/or the wider community sector, with experience working in the NDIS sector and sound case management practices
  • Skills and experience to develop support plans (and strategies) which assist participants to achieve goals in their NDIS plans and that support participants with recovery and improved health outcomes
  • Experience working with people with complex needs and engaging vulnerable clients whilst maintaining clear boundaries and accountability
  • Excellent communication (verbal and written) skills
  • Clear and Current National Police, Working with Children and NDIS Worker Screening Checks
  • A current valid Driver's License and the ability to undertake travel for the role
  • Strong Microsoft Office skills and capacity to use case management software (SupportAbility)
  • A strong service orientation and a willingness to pitch in and help others.

Experience and relevant tertiary qualifications (Certificate IV or above) are required.
