The Community Mental Health Service is committed to the provision of quality health services to the mental health community. We aim to deliver mental health services which, through research and innovative commitments, set standards that are emulated by other national and international organisations.

We aim to lead through achievement and innovation in clinical practice, research, teaching, training and management. We are committed to developing the highest skills in our community mental health staff through the LHD’s pioneering Diploma of Mental Health program, structured workplace competency development program, and supervision programs.

What you'll be doing

Provide Senior Social Work clinical leadership within the Community Mental Health Core Team, providing social work services including optimisation of patient/client care delivery, as well as supporting the mentoring and development of staff.

The Community Mental Health/Core Team model of care states: 

The “Community Mental Health Team” (Core Team) is the mainstay of the community mental health system. Core Teams and provide a range of clinical services including intake, assessment, consultation, treatment ,and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care, and short term care coordination.

Consumers under the care of Core Teams will either have:

  • Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months.
  • A period of treatment as recommended by evidence based practices.
  • Ongoing care and monitoring for prolonged periods

The requirements

  • Must hold a Bachelor or post graduate degree in Social Work that provides eligibility for full membership of the Australian Association of Social Work (AASW) with experience at Level 2 or higher.
  • Current NSW Driver's Licence (P1 and P2 Acceptable).
  • Able to work Monday to Friday 8:30am to 5:00pm
