Welcome to St John of God Health Care

A flagship program of St John of God Social Outreach, Horizon House provides young people aged 16-22 years who are currently experiencing, or are at serious risk of, homelessness with stable long-term accommodation and support to help them achieve independence.

Horizon House delivers early intervention and case management strategies that focus on supporting residents to set and achieve education and/or vocational goals. All young people who live in a Horizon House must be engaged in education, training or employment as part of their journey towards independence and commit to participating in a life skills program. We also provide individualised case management, general counseling and help to reconcile young people with their families when it’s safe and appropriate to do so.

The Position

St John of God Social Outreach has an exciting opportunity available for a Support Coordinator to lead our dynamic Horizon House located in Geelong. The successful candidate will be responsible for managing all aspects of the House, including its team of residential youth workers.

This position is being offered on a permanent full time contract, working 76 hours per fortnight from Monday to Friday. The successful candidate will also be required to participate in the management after-hour’s on-call roster.

Reporting to the Youth Services Area Coordinator, the successful candidate will be joining a national team of enthusiastic and committed Support Coordinators. The successful candidate will also:

  • Provide therapeutic and practical support to residents.
  • Oversee the delivery of an Independent Life Skills Program which helps young people living at the house develop essential skills such as cooking, cleaning and personal budgeting they’ll need in order to live independently.
  • Provide outreach support to young people living in transitional accommodation (outside the Horizon House) – many of these young people will be former Horizon House residents who have taken the next step on their journey towards independent living.
  • Respond to critical incidents.
  • Increase awareness and understanding of Horizon House among relevant community groups and support agencies.

All applicants must have a tertiary qualification from a recognised University and/or TAFE College in Youth Work, Social Work or Psychology and a minimum of 5 years’ experience in provision of direct therapeutic care to at-risk young people in a residential care setting. Experiencing managing a team would be highly advantageous.

The successful candidate will have:

  • A willingness and capacity to embrace the Mission and Values of St John of God Health Care.
  • Demonstrated ability to take initiative, work independently, and provide leadership.
  • Demonstrated experience in the provision of complex case management and managing challenging behaviours in young people, and the ability and to provide sound advice and professional development to staff within a therapeutic context.
  • Demonstrated training and experience in the provision of professional and effective supervision of staff.
  • Experience in program supervision, coordination and/or development.
  • High level of verbal and written communication skills and computer literacy.
