The Opportunity

The ‘Succeed Differently Beyond Blindness’ rapid-response peer support program for people with a recent diagnosis of vision loss. Support during the most vulnerable time after vision loss diagnosis.

The program will recruit and train a specialised workforce of peer support workers with lived experience of vision loss, who will deliver a rapid response service via telephone.

As the Program Lead you will report to the Field Services Manager and coordinate the program in line with Grant funding and requirements. This is a fixed Term contract to commence ASAP until the 30th June 2027 (as per grant agreement).

Key Responsibilities:

  • Oversee recruitment of Peer Support Workers in line with the project activity requirements.
  • Attend with, supervise and ensure all Peer Support Workers undertake the bespoke training program and respond appropriately to situations where a Peer Support Worker may not be exhibiting the required characteristics and behaviours for the role.
  • Manage the client referral, worker allocation, scheduling, goal setting and service delivery, recording of client service entries in the CRM, exit at client direction and follow up stages of the client journey through the program.
  • Within the Community Services Department, promote and respond to queries about the Succeed Differently Beyond Blindness Peer Support Program, for existing clients experiencing a significant change in diagnosis of vision loss, for referral to the program.
  • Maintain regular contact with the staff undertaking business development activities, to ensure sufficient supplies of marketing collateral and regular promotion of the Succeed Differently Beyond Blindness peer support program is undertaken with client referral sources.
  • Maintain regular contact with and support to Peer Support Workers, and offer Employee Assistance Program where appropriate, and/or counselling support following difficult client interactions.
  • Monitor client progress via goal achievement and seek feedback from clients on their experience of the program and opportunities for improvement.
  • Compile and maintain the ‘knowledge bank’ of information accessible by Peer Support Workers and disseminated to clients, regarding support services, government agencies, mental health services, vision specific supports and adaptive technology.

Your Skills and Expertise

  • Minimum Certificate IV in Aged Care, Disability, Community Services or other Humans Services discipline
  • Previous experience in managing/leading teams
  • Confidence and ability to manage a remote workforce
  • Previous experience in managing projects and meeting milestones
  • Ability to capture, interpret and report on data
  • Proficiency in the use of Microsoft Office Quite and Client Management System
