Plan Navigators started in mid-2018 as a division of Inclusion WA called Personal Assistants WA. We set up the service to assist people with disability to take an increased level of control of their funding and their services. Growing out of Inclusion WA, Plan Navigators draws on the 30 years of experience of walking beside people as they become connected to their communities.

Plan Navigators is currently supporting people who are the employer of their own staff to recruit for Passionate, Creative and Intelligent Mentors to work alongside a diverse range of people.

Katie* is 21 and a lot of fun to be around, she has an amazing sense of humour, she lives in her own home with her Dachshund Lynk. There are several positions one of which requires Full time live in support for 10 days a fortnight (being home from 5pm - 9am the following day. Other mentor positions are available for day time shifts throughout the week.

Having a chronic condition along with Autism Katie spends much of her day dealing with physical and sensory issues that require support. Her capacity to cope varies depending on her pain levels and the successful applicant will need to be intuitive to Katie’s needs and undertake day to day tasks with a degree of common sense. You will have down time in this position that will enable you to have time to yourself as long as Katie’s needs are prioritised and you act in her best interest.

Responsibilities

  • Household tasks including cleaning, laundry, gardening etc
  • Food shopping and meal prep and cooking
  • Looking after Lynk, feeding, cleaning up after him and walking him
  • Assisting Katie to attend appointments
  • Prompting Katie to undertake personal care activities
  • Provide assistance for Katie to use her wheelchair or walking stick in the community
  • Monitor Katie for signs of agitation and increased anxiety
  • Record Katies medication usage
  • Supervise use of heat packs

Desirable but not required

  • Knowledge of females on the Autism Spectrum
  • Knowledge of complex pain disorders and chronic health conditions

Required

  • Drivers Licence ‘C’ class
  • First Aid certificate with CPR
  • NDIS workers screening check
