The Wish Group is an organisation that specialises in mental health, disability and training. Owned by a mental health nurse we have been in operation for 30 years. We are a small but experienced team that look after staff as well as we look after our participants and clients. We provide excellent support to our staff, with small teams and small caseloads. Qualifications & experience

• 1 year's experience as a NDIS support coordinator.
• Qualifications within the Community Services, Mental Health and Disability/and or Similar.
• Excellent timekeeping and administrative skills
• Highly developed communication and writing skills. Experience working with complex mental health and disability presentations
• Current Drivers licence and car with comprehensive insurance
• Great work ethic and flexibility

Tasks & responsibilities

• Work with vulnerable participants and their families to interpret and understand their NDIS Plans
• Build strong working relationships with participants, their families and key stakeholders to ensure a holistic approach to their care
• Work in collaboration with participants to find appropriate services that match their individual needs
• Develop community transition plans and work with participants to implement NDIS services
• Assist participants to build their capacity in line with their plan goals.
• Work with our team of support coordinators and specialist support coordinators to find the best outcomes for the participants.
• Complete reports and reviews as required
