At North West Disability Services (NWDS) we believe in the fundamental right of people with disabilities to live a full and valuable life within the community. Located in various sites throughout North West Sydney, we provide accommodation services, life skills training, respite care, children’s services, individual support and support coordination.

Position Overview

We are looking for a support coordinator to join our team on a full-time basis. Our support coordinators assist participants to achieve their plan goals by providing assistance to choose and coordinate providers, develop service agreements, manage plan funds, resolve queries or concerns that may arise, and develop goals for the future. The support coordinator will find and link with appropriate services, organisations and activities in the community whilst building the skills of participants and carers to self-manage. They utilise the participant portal and report back to NDIA as required.

This is a permanent role, working flexibly to suit the needs of the current participants with support coordination in their existing plan. Hours and days of work will be flexible to meet the changing needs of the participants and carers from Monday to Friday.

To be successful in this role you will ideally have:

  • Ability to creatively develop an outstanding NDIS Plan strategy with participants and carers and support its implementation
  • Ability to monitor and maximise participants NDIS budgets
  • Commitment to achieving the very best for people with a disability
  • Knowledge of the Disability Inclusion Act & Disability Service Standards
  • Current Working with Children Check and NDIS worker check
  • First Aid Certificate and LR Driver License (desirable)
  • NDIS Workers Orientation module completed
  • Certificate 3 and / or Certificate 4 in Individual Support (Disability)
  • Own vehicle with comprehensive insurance
  • Outstanding ability to communicate well verbally and in writing
  • High level administrative and financial record keeping skills
  • Ability to work autonomously
  • Knowledge of the NDIS and its processes
  • Good teamwork and interpersonal skills
