Diabetes Nurse Educator role (Permanent Part-Time, 0.5 EFT) provides culturally safe, high quality, evidence-based assessment, education and support to clients for the self-management and prevention of diabetes.
Roles and responsibilities include, but are not limited to
- To provide high quality and person centred diabetes prevention and self-management education services to individuals and groups, and particularly to members of identified target groups and communities
- Liaise with and support internal and external health service providers and other health services in the management of their clients with or at risk of diabetes, and assist in coordination of services to clients
- Maintain high quality, accurate client records as per AHPRA and NRCH clinical record keeping guidelines.
- Maintain privacy and confidentiality in regard to client records and information.
- Attend and contribute as appropriate at clinical meetings, peer review, oral health and site meetings.
- Meet service objectives, standards and targets; ensure Quality Assurance in relation to direct patient care; continuously improve services provided.
Essential Qualifications and Attributes include, but are not limited to
- Current registration with AHPRA as Registered Nurse (Division 1).
- Graduate Certificate in Diabetes Education or equivalent or higher qualification.
- Holding or working towards Credentialed Diabetes Educator status with Australian Diabetes Educators Association
- Current Professional Development requirements as per APHRA.
- Demonstrated competence and skill as a Diabetes Educator with a minimum two years’ experience.
- Demonstrated experience with clients requiring insulin stabilisation.
- Understanding of the principles of self-management of chronic disease.
- Knowledge of community health principles and a commitment to providing primary health services in the community.
- Experience working with and appropriately responding to people from Culturally and Linguistically Diverse Backgrounds and with complex health, mental health and social issues.
- Demonstrated ability to work within an Empowerment or Health Literacy model to support clients in making informed choices regarding their health and self-management of diabetes.
