MultiLink Community Services is an incorporated association providing coordinated, creative and responsive programs to meet the needs culturally and linguistically diverse communities across South East Queensland. Our services are delivered across the life span providing youth, settlement, employment & training and other services through to aged care. About the role
The Queensland Community Support Scheme facilitator will be responsible for the delivery of care and support services to QCSS eligible consumers by undertaking intake, establishment, monitoring and review of support plans as required by the program guidelines, individual consumer needs and Quality Assurance frameworks. The position ensures that the abilities and choices for care are met through a person centred, enabling and capacity building approach through the provision of time limited goal orientated services. This is a full-time position Monday to Friday 8.30 – 5.00 located at our Logan Central offices.
Selection Criteria
- Qualifications from a recognised tertiary institution in Community Services, or similar discipline and/or a minimum of 3 years’ experience in provision and coordination of services for consumers under 65 years of age. (previously known as QCC)
- Demonstrated comprehensive knowledge of Consumer Directed Care facilitation, time limited goal orientated services and the wellness and reablement environment
- Strong relational customer service mindset with demonstrated ability to communicate effectively with people from diverse language, faith and cultural backgrounds
- Demonstrated experience of complex case management & care coordination
- Demonstrated experience working with a highly diverse team, exhibiting a holistic approach to the delivery of service through working harmoniously and constructively in a team setting
- Demonstrated well developed time management and organisational skills with the capacity to work effectively within tight time constraints
- Strong interpersonal communication skills
Other Requirements
- Possession of a current Queensland “C” class driver’s licence is essential, with a good driving record
- Successful applicants will be required to possess:
- a current National Police Check (undertaken within the last two years)
- a current Blue Card and Yellow Card (or be willing to obtain)
- Rights to Work in Australia evidence
