About the Role

The CHSP Home Care Facilitator is responsible for providing superior customer service and delivery of client focused care and services that effectively meet individual client needs, while promoting functional independence, wellness and lifestyle choices within a Consumer Directed Care (CDC) environment. Working in partnership with consumers and carers receiving CHSP services, the position will co-design a plan of care based on the consumer’s goals, preferences and assessed needs. Working within a strong customer service model, the facilitator will liaise with internal and external stakeholders to ensure positive consumer outcomes within the resource allocations available within the program. The Facilitator is responsible for the supervision of Support Workers in the delivery of consumer care and services.

Selection Criteria

  • Qualifications from a recognised tertiary institution in Aged Care, or similar discipline and/or a minimum of 3 years’ experience in provision and coordination of aged care services to consumers
  • Demonstrated comprehensive knowledge of Consumer Directed Care facilitation and the wellness and reablement environment
  • Strong relational customer service mindset with demonstrated ability to communicate effectively with people from diverse language, faith and cultural backgrounds
  • Experience of case management/coordination advantageous
  • Demonstrated experience working with a highly diverse team, exhibiting a holistic approach to the delivery of service through working harmoniously and constructively in a team setting
  • Demonstrated well developed time management and organisational skills with the capacity to work effectively within tight time constraints
  • Strong interpersonal communication skills

Other Requirements

  • Possession of a current Queensland “C” class driver’s licence is essential, with a good driving record
  • Successful applicants will be required to possess:
    • A current National Police Check (undertaken within the last two years)
    • A current Blue Card and Yellow Card (or be willing to obtain)
  • Rights to Work in Australia evidence
