About Subacute GEM Rehabilitation at Monash Health

Monash Health is an impressive organisation of seven public hospitals, 21 community health sites, five aged care facilities, university affiliated international research and tertiary education. We provide healthcare for the entire lifespan from pre-birth to palliative care and all stages in between of clinical care.

The Subacute GEM Rehabilitation Wards (South 2, 5 & 6) at The Kingston Centre (Cheltenham) is a secure, Sub-Acute Geriatric Evaluation Management Ward providing Specialist assessment, treatment and management of patients with complex and multiple medical, functional, and often cognitive conditions. We provide care and assistance with recovery for a diverse range of geriatric patients who have been in hospital and require complex interdisciplinary interventions.

We are dedicated to delivering patient care in line with our iCare values and that all patients are treated with dignity and respect as they transition into longer term Residential care or back into their home environment.

About the Role

We currently have vacancies within Wards 2, 5 & 6 for Registered Nurses, each to work through a rotating roster consisting of 7 days, all shifts, amongst our dedicated team.

The capacity of work to undertake complex discharge planning, complex wounds, patient training, address continence and catheter issues. With the overall aim to see that all patients are treated with dignity and respect as they transition into longer term Residential care or back into their home environment.

About You

Understanding the importance of patient centred care, you have experience within an acute/subacute hospital ward and have a genuine interest in working and interacting with our diverse patients who, at times, can demonstrate challenging behaviours due to previous acquired brain injuries.

Your critical thinking, willingness to learn and ability to work cohesively and share knowledge within a team will see you excel in this role whilst we provide you with guidance, support and training to learn the rhythm of rehabilitation.

What you need:

  • Current AHPRA registration - as a Registered Nurse
  • Proven clinical experience working within aged or rehabilitation responsibilities
  • Flexible working style to adapt to changes through the day
  • Compassionate manner, advocating for patients in their vulnerable state
  • Relevant working visa
  • Employee Working with Children Check
