We are looking for Registered Nurses to work as Care Coordinators to support the most vulnerable people in our Community. We support people in the self-management of their health conditions including chronic heart failure, COPD, asthma and complex biopsychosocial needs. This role is integral in the assessment and management of client's wellbeing, and operates from a person - centred care framework aligning with the social model of health.
