About The Role

We are looking for Registered Nurses to work as Care Coordinators to support the most vulnerable people in our Community. We support people in the self-management of their health conditions including chronic heart failure, COPD, asthma and complex biopsychosocial needs. This role is integral in the assessment and management of client's wellbeing, and operates from a person - centred care framework aligning with the social model of health. With a high level of attention to detail, have exceptional clinical expertise, knowledge and skill

About You

  • You are a dynamic Registered Nurse. With a genuine passion and interest in this unique opportunity
  • We work mainly Monday to Friday with some coverage required on weekends and public holidays
  • As you will work across various Monash Health sites, and home visiting a valid driver’s licence is essential.

Looking after Our People

  • We strive to ensure that all Monash Health work in accordance with all relevant Occupational Health and Safety, Infection Prevention & Respiratory Protection Program procedures
  • Established escalation pathway to Nurse Practitioners and Medical Professionals when required
  • Completing all mandatory and competency assessment training

What you Need

Essential:

  • Current AHPRA registration
  • Demonstrated recent experience as a registered nurse.
  • Valid Driver's licence

Desirable:

  • Previous Community Nursing experience
  • Post-graduate qualifications
