Monash Health Community is recruiting a Community Health Nurse to offer victims of sexual assault and their family’s access to holistic health care. Working across Dandenong and Seaford Multidisciplinary Centres, you will work collaboratively with key service providers including CASA counsellors, Victoria Police, and Child Protection to improve the health care of vulnerable individuals.

Monash Health is an impressive organisation of seven public hospitals, 21 community health sites, five aged care facilities, university affiliated international research and tertiary education. We provide healthcare for the entire lifespan from pre-birth to palliative care and all stages in between, and we have recently opened the new 230-bed Monash Children’s Hospital.

About the Role

The Multidisciplinary Centre (MDC) Community Health Nursing service will be provided on an in-reach and outreach basis from each MDC. The CHN will undertake initial needs identification and provide direct nursing services to victims/survivors of sexual assault and family violence.

The MDC nurse will identify client health needs via a holistic health assessment, develop and implement client centred care plans, provide health education, increase health literacy, and assist clients to transition into universal health care.

Excellent clinical skills will ensure the delivery of best-practice health care services to sexual assault survivors with a particular focus on assessment and care coordination.

This position will be primarily based in Melbourne’s South Eastern suburbs and will require travel between Dandenong, Seaford and Springvale The MDC CHN will:

  • Advise victim/survivors on the health implications of sexual assault
  • Perform health assessments and facilitate their access to other health services as appropriate
  • Provide individual health and wellbeing support, advocacy and nurse-specific counselling, educate various stakeholders about health issues affected MDC clients
  • Generally raise awareness about the role of the MDC and CHN role
  • Establish effective local health care networks with the objective of improving the care of victim/survivors of sexual assault and contribute to the effective operations of the state-wide MDC nursing network

About you

In the role of MDC Community Health Nurse you will demonstrate the ability to work autonomously, demonstrating the ability to prioritise tasks. You will possess excellent clinical and communication skills with a particular interest in contributing to the development of an innovative model of care and drive initiatives, support relationship development and collaboration.

What you need

Essential

  • Registered Nurse Division 1
  • Current registration with Australian Health Practitioner Regulation Agency (AHPRA)
  • Minimum of 3 years' post-basic registration clinical experience
  • Sound knowledge of health system and the social model of health and a commitment to improving integration of care across the care spectrum
  • Broad understanding of the needs of victims of sexual assault, including family members, and extensive experience in working with this client group
  • Excellent interpersonal skills, mediation, and liaison skills
  • Ability to engage, build rapport and collaborate with a diverse range of people including staff at all levels, patients, carers and consumers to build effective relationships with key stakeholders.
  • A high degree of computer literacy including the Microsoft Office suite including Word, Excel, PowerPoint and Outlook and the willingness and skills to learn new programs quickly.
  • Current AHPRA registration
  • Working with Children’s Check
  • Proven clinical experience

Desirable

  • Qualifications and/or experience in primary health, community health, health education, case management or women's health, family violence, mental health risk assessment
  • Additional relevant nursing qualifications in:
    • Sexual and reproductive health
