The Atherton Road Community Mental Health Service comprises a multidisciplinary team of case managers including Occupational Therapists, Social Workers and Registered Psychiatric Nurses and also supported by a range of Clinicians including Consultant Psychiatrists, Junior Doctors, Patient Transition Co-ordinators and Clinical Psychologists. We encourage clients to attend the clinic where required and in their own home. The service, which operates Monday to Friday 8.30 – 17.00 aims to provide comprehensive mental health care that considers all aspects of a person’s needs, encouraging the support and involvement of the carer or family members.
We are excited to announce that we have several positions available now for experienced and passionate Clinical Case Managers. We have two Full Time permanent ongoing roles (76 hours p/fortnight) and one Part Time permanent ongoing position (64 hours per fortnight).
Mental Health clinicians are required to provide the best care to our communities. Comprehensive mental health care means that all aspects of a person’s needs are considered when providing that care. Working in line with Monash guideline and procedure, the role of the case manager is to undertake holistic assessment of needs, treatment planning, monitoring, advocacy and linking of the consumer with appropriate community support services.
As a Case Manager you will work collaboratively with other clinicians to ensure high quality of care is provided to consumers in the service area. You will look to you to provide comprehensive mental health assessments and demonstrate competence in managing complex, mental health issues whilst adjusting to changing priorities. You will be required to manage an allocated caseload of clients, providing direct support and interventions, co-ordination of care and referral to other relevant community professionals, agencies and services.
As a clinical role it will draw on your knowledge in mental health legislation and policy and expect you to demonstrate your expertise in mental Health and clinical skills, all while undertaking a clinical caseload including duty, mental state and risk assessment and crisis management. Client contact may occur at the clinic, via teleconference or at their own accommodation as required.
