A Day in the Life

A unique opportunity exists for a talented healthcare professional from an Allied Health or Nursing background, to join our Home Based Rehabilitation Program as Community Rehabilitation Case Manager in a permanent, part time capacity. You will be working in a variety of specialty areas including (but not limited to): Neurological, Orthopaedic and General Rehabilitation. Working with a diverse range of clients, no two days will be the same! You will work alongside our talented wider interdisciplinary team.

The Community Rehabilitation Case Manager supports client flow within the community rehabilitation program and facilitates the successful transition of referred clients from hospital to home and toward community reintegration. As the key contact to the client, team, and key stakeholders in a variety of settings, you will complete comprehensive assessments in collaboration with clients and the interdisciplinary team and coordinate care plans to ensure high quality client-centred care. This role is based in the Cranbourne Integrated Care Centre and will see you;

• Advocate for your clients within a large multidisciplinary team
• Facilitate the successful transition of referred clients from hospital to home and toward community reintegration
• Coordinate client-centred, goal driven rehabilitation services to maximise a client’s independence, health and well-being following acute illness or injury

About You

We are looking for an energetic and compassionate professional from any Allied Health background or Nursing who is looking for a new and unique challenge. You will enjoy a diverse and varied role that offers you a dynamic and rewarding career. You will;

What you need:

This unique role is open to anyone from a professional Allied Health or Nursing background who has exposure to working within a multidisciplinary team and who has relevant leadership and management skills.

• Allied Health or nursing qualification or equivalent in line with relevant discipline requirements
• Relevant clinical and leadership experience in a rehabilitation or sub-acute setting
• Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) or Registered with Nursing and Midwifery Board of Australia via AHPRA
• Current Drivers Licence
