A unique opportunity exists for a talented healthcare professional from an Allied Health or Nursing background, to join our Home Based Rehabilitation Program as Community Rehabilitation Case Manager in a permanent, part time capacity. You will be working in a variety of specialty areas including (but not limited to): Neurological, Orthopaedic and General Rehabilitation. Working with a diverse range of clients, no two days will be the same! You will work alongside our talented wider interdisciplinary team.
The Community Rehabilitation Case Manager supports client flow within the community rehabilitation program and facilitates the successful transition of referred clients from hospital to home and toward community reintegration. As the key contact to the client, team, and key stakeholders in a variety of settings, you will complete comprehensive assessments in collaboration with clients and the interdisciplinary team and coordinate care plans to ensure high quality client-centred care.
