Our Journey

The Monash at Home – Aged and Rehabilitation Care service, previously GEM at Home, launched in December 2020 and is a fundamental part of the broader Aged and Rehabilitation Division, providing safe and high-quality home-based subacute level care for individuals with complex medical, functional, and often cognitive issues, who require comprehensive, medically led multidisciplinary assessment and optimisation.

The service is seen as an alternative to hospital-based subacute care, designed to facilitate an earlier discharge from acute care and/or to avoid a subacute hospital stay entirely. Patients within the service receive at least daily visits from a member of the multidisciplinary team and access to 24-hour telephone support. The model of care aims to provide short term, high quality, safe and effective patient-centred care to individuals within the primary Monash Health catchment. The Program is entering an exciting phase of growth which will include, but not limited to, increasing the clinical service capacity and extending the geographical catchment.

A Day in the Life

The Grade 3 Allied Health Assistant (AHA) with the Monash at Home – Aged and Rehabilitation Care service, is responsible for providing strong leadership to the AHA team and actively contributing to the leadership group and promoting the profession both internally and externally to the organisation. The AHA facilitates independence, assists the team to ensure safety and quality of life of clients through a commitment to client focused, excellent care, collaborative practice and evidence-based knowledge. The AHA is responsible for working with the inter-professional team to provide this care through creating strong partnerships within and outside of Monash Health. Key elements of the role include:

  • Provide strong leadership to the Monash at Home – Aged and Rehabilitation Care AHA team in areas of workload, quality improvement and professional development
  • Represent the Monash at Home – Aged and Rehabilitation Care AHA team on the Monash Health AHA Leadership Group
  • Work with minimal supervision to implement therapeutic and related activities
  • Demonstrate a sound knowledge of medical conditions, terminology, disease, and ageing process.
  • Understand the basic theoretical principles of the work undertaken by their supervising Allied Health Professional

About You

You are passionate AHA with exceptional clinical knowledge and assessment skills, and ability to work with a range of stakeholders. You demonstrate this passion in delivering clinical care in the community setting and through working within a team in this field. You have a proven ability to deliver successful, timely outcomes across service delivery, and you are an effective communicator who builds rapport rapidly, and whilst you can work autonomously, you are also a strong team player.

What you need:

  • Certificate IV Allied Health Assistance or equivalent as outlined in the Monash Health Credentialing Standards or in consultation with the Allied Health Assistant Advisor
  • Greater than 2-4 years AHA experience in a hospital setting
  • Current driver’s licence without restriction
