Your Opportunity

Mission Australia currently have opportunity to join us as a Case Manager for our Northern Sydney Specialist Homelessness Support Service. This position will provide specialist welfare support to people who are experiencing homelessness and other related issues.

Your key responsibilities will be to:

• Respond to referrals to the service, and conduct thorough assessments to determine their eligibility and support needs and establish a case plan with the client and the other relevant stakeholders
• Contribute to the development and facilitation of group sessions to meet the needs of the service, clients and the local community
• Develop, implement and review individualised case plans for each client, addressing such issues as living skills, welfare needs, interpersonal skills, health, education, training, employment options, social skills, recreational needs and family relationships
• Establish record, monitor and review each person’s progress, in consultation with Program Manager
• Develop quality relationships with a range of key stakeholders including community organisations, community housing providers and health professionals to allow for the effective provision of support and information to clients

Requirements for Success

• Relevant tertiary qualification with at least 2 years’ experience
• Demonstrated ability to provide specialised or complex therapeutic interventions and manage a caseload of complex consumers.
• Experience with clients dealing with issues including homelessness, poverty, domestic violence, mental health and alcohol/drugs misuse, and knowledge of support services
• Highly developed administrative skills along with well-developed written and oral communication

Before starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people.
