Your Opportunity                    

In this role you will support young people to transition from Triple Care Farm Withdrawal program to further treatment or to the community. You will provide ongoing community support for young people for up to six months.

Your key responsibilities will include:

  • Assess the placement needs of young persons including ongoing treatment accommodation, employment, training, social and emotional needs
  • With the young person, develop an individual case plan that identifies the long term placement needs and provide practical support required to follow up the goals identified
  •  Assist young people to identify treatment and/or accommodation choices, supporting young people to identifying where they would like to go after completing their program
  • Assist in the placement needs of each young person including referral onto further AOD treatment services, accommodation, training, education, and other special support services
  • Participate in relevant Case Conferences and meetings to contribute to the ongoing assessment, linking placement goals to the Individual Case Plan and encouragement in transitioning towards placement and integrated care
  • Maintain regular contact with young people in the aftercare program and with key stakeholders identified in their individual case plan. This contact includes face to face, telephone, email and social media as identified in individual case plans
  • Partnerships are developed with service providers across a spectrum of service types to be able to link young people with services in the community. Where appropriate formal partnership agreements are developed

Position Requirements

To be successful in this role, you will need to possess:

  • Certificate IV or equivalent or higher qualification in the field of youth work, Alcohol and Other Drugs, Mental Health or relevant health/human services field
  • Demonstrated experience in case management
  • Knowledge of mental health issues, substance use treatment and working with youth as well as the non-government alcohol and other drugs, mental health and youth sectors
  • Proven ability to relate to young people with complex needs
  • Proven ability to network with other agencies
  • Fluent computer skills in Microsoft Office packages
  • Current unrestricted NSW Driver’s license

As part of our Employment screening process you will be required to undertake a National Criminal History Check.
