About the role

The Adult Prevention and Recovery Care service (APARC) is a residential program providing services to clients aged between 16-65 years which utilises a recovery-focused, clinical treatment framework. APARC is designed as a ‘step up, step down’ program for people with mental illness who require individualised treatment and support when entering either from the community or transitioning from acute inpatient care.

We are seeking a compassionate, self-motivated Community Mental Health Practitioner - Lead Practitioner for a permanent, full time opportunity to work alongside a caring team in the Deer Park area. You will lead and mentor a team to deliver high quality recovery orientated, person-centred services to clients to support recovery from mental health challenges, achieve life goals and live independently.

Key responsibilities

  • Coach and mentor the team to deliver quality services and facilitate positive educational and social experiences to support clients.
  • Support clients to return to their community and enjoy a meaningful life through staged approach to recovery.
  • Provide leadership and work as part of a multidisciplinary team offering specialised psychosocial support.
  • Build rapport and develop trusted relationships with clients, families and carers.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to the client’s recovery goals.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Proven capability to implement therapeutic, recovery-oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
