About the role

The Housing & Accommodation Support Partnership/ Supported Independent Living (HASP SIL) program is a 24/7 residential service for people living independently with mental illness and supports clients with individualised services provided in line with their NDIS plan and unique needs. The program aims to promote greater self-management of mental and general health, increases access to the community and develops an individual’s capacity to live independently with a focus on achievement of their better life goals.

We are seeking a compassionate, resilient Community Mental Health Practitioner for a permanent, full time opportunity to work on a 24/7 rotating roster in the Burnside area. You will support clients to achieve their recovery goals through one-to-one support, facilitating peer learning workshops and assistance with daily tasks in line with Mind’s Model of Recovery Oriented Practice and My Better Life model.

Key responsibilities

• Provide support to clients using targeted and integrated case managed responses.
• Deliver high quality services and facilitate positive educational and social experiences.
• Work collaboratively with a range of services and the community to ensure a co-ordinated, integrated response to the client’s recovery goals.
• Plan and deliver group programs that aid clients to build their skills and focus on recovery.

What you’ll bring

• Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
• Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
• Demonstrated understanding of the Community Services sector and ability to build strong relationships with agencies, clinical providers and key stakeholders.
• Relevant experience in facilitating group activities for clients experiencing mental ill health.
