Mind is looking for a Community Mental Health Practitioner who is also experienced in the area of disability. This role will be based at The Supported Independent Living (SIL) program in Preston. The service provides recovery focused support to people with mental ill health. Within this residential environment, clients can learn skills and manage their mental and physical health within a safe, supported environment.

As part of the team at Mind you will:

• Work full time, 38 hours per week on a fixed term until 30 June 2019
• Work within a 24 hour 7 day per week rotating roster including weekends and sleepover shifts
• Support dual disability customers to achieve their recovery goals through one-to-one support, facilitating peer learning workshops and assistance with daily tasks
• Develop individual program plans that outline goals, aspirations and desires working towards recovery and independent living
• Work as a member of a dynamic and committed team to support customers through recovery orientated practice and by providing Positive Behaviour Support
• Support residents and assist in the development of daily living tasks including housekeeping, shopping and cooking

You will have skills in:

• Tertiary qualifications (minimum certificate IV) in Mental Health, Disability, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind
• Previous experience in mental health and specialises in the area of dual disability
• Managing emergency situations and a solid understanding of risk management
