THE ROLE
Community Connections is a unique day program that connects people with people. With three service sites based in Lilydale and Mt Evelyn, Melba tailor programs and activities to people living with a disability that are value-based, with the focus being on each person and the support they need to achieve their NDIS goals.
Melba offers centre-based group programs or 1:1 support arrangement with experienced and passionate support staff. Melba assists people to develop a range of skills and support each person to make choices in their life and to have control over their decision making.
The Outcomes Lead, mentors & guides a team of Disability Support Workers to deliver high quality, person-centred and participant focused outcomes for people supported:
- Understanding, developing, and refining individual programs of support within a set budget and in line with a person’s NDIS goals
- Developing links to a person’s community that offer that person greater opportunity for inclusion and the development of valued social roles
- On-boarding, support, supervision, and development of assigned staff
- Supported Person transport organisation during service delivery
- Student placement supervision as required
- In collaboration with the Participant Engagement & Quality Compliance Coordinator, responsible for service quality monitoring, planning, analysing, and reporting
- Collaborating with the Roster Administrator, responsibility for permanent rosters and system updating
- Working in conjunction with other Outcomes Leads, Support Coordination and Participant Engagement & Quality Compliance Coordinator to maintain person supported information and documentation
- Review and approve Carelink entries, rejects, clash lists etc. in line with payroll and billing requirements
- Deliver support – 40% of your time will be direct disability support work
This is a permanent full time role required onsite from Monday to Friday.
About you
You’ll be driven and show genuine passion for building capability within your team. In addition, you:
- Are a natural influencer, modelling a positive attitude, professional conduct and unwavering commitment to quality outcomes and service delivery for staff, individuals with disability and the community.
- Possess an understanding of the NDIS, it’s funding and systems.
- Can offer experience leading dynamic teams and the recruitment and rostering of staff.
- Have prior support worker experience including demonstrated understanding and knowledge of the tasks of a disability support worker.
- Can demonstrate commitment to service excellence, human rights and zero tolerance to violence, abuse, neglect, and exploitation.
- Experience in running a day service is desirable, but not mandatory.
