Multicultural Care is looking to recruit an experienced Aged Care Advisor to work within our team in coordinating and growing Multicultural Care’s Short Term Restorative Care program. We are looking to recruit someone for this position immediately to start working with us in our new offices in Burwood!

The Role

This position is a key member of the Client Services Team, the Aged Care Advisor position is responsible for the provision of case management services for clients across the STRC programs by undertaking assessments of each client in collaboration with the client and their family/ carer, determining a number of health and well-being maintenance and promotion strategies as well as assisting the client to maintain a quality of life.

This includes;

  • Ensuring participants receive support that is appropriate to their individual interests, goals, capabilities, and support needs.
  • Consulting with and facilitating involvement of all partners in the assessment and individual planning process.
  • Visiting new clients to complete Client Assessments in consultation with the client to inform the development of the clients' budget.
  • All client communications are accurately reported and recorded in a timely manner.
  • Complete Work Health and Safety assessment for all clients.
  • Regularly monitoring and updating of the Clients service Care plan.
  • Responsible for the client's administration management
  • Ensure operations are conducted ethically, fairly and within the statutory legal and contractual requirements.

About You

This is a crucial role with a high-level community partnering through frequent engagement, communication, and evaluation of programs. To be successful in this role you will have:

  • Degree in Allied health is required.
  • Registration with AHPRA is essential.
  • Demonstrated experience in Case Management.
  • Experience working in the aged care or disability sector.
  • Demonstrated understanding of needs and issues of people with disability from CALD backgrounds.
  • Demonstrated experience in needs assessment and management of the elderly.
  • Demonstrated ability to supervise and develop out of office staff and speak a second language.
  • Understanding of work health safety in home and community settings.
  • High level of communication and ability to work collaboratively is essential.
  • Experience in Proda, MAC & Alayacare is highly regarded.
