We are North Western Melbourne Primary Health Network (NWMPHN), Victoria's largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care within our community. As a commissioning organisation, NWMPHN allocates funds to address community health needs, and works in partnership with a large range of service delivery and community organisations to deliver flexible and tailored solutions to address the health needs of our communities. Commissioning also provides the mechanism for performance evaluation and accountability back to communities and to governments through setting and monitoring the performance of funded health care providers.
The primary focus of this role is to support the regional coordination and oversight of primary health care initiatives for young people in NWMPHN and within Victorian schools. Programs of interest include the Doctors in Secondary Schools program and the Enhancing Mental Health Supports in Secondary Schools initiative.
Working with service providers, ensure effective monitoring and evaluation strategies that drive efficiency, effectiveness and quality, and that contribute to the evidence base are in place.
Implement and utilise project management approaches to ensure the delivery of work plans and priorities.
Identifying key stakeholders, client groups and other key stakeholders to build collaborative partnerships to drive the work.
Qualifications or relevant experience in a health, education or related field relevant to the position or equivalent experience
