About the role

Reporting to the Manager, CAREinMIND Mental Health Services, the Mental Health Care Navigator - Intensive Support Service (ISS) is primarily responsible for the intake and assessment of ISS referrals for people who have severe and complex mental health issues. As a member of the CAREinMIND team, you will support the intake and clinical assessment of individuals that have been referred to NWMPHN funded mental health programs. Additionally, you will work strategically to contribute to the development and review of the team's performance targets, and to support the promotion and representation of our services and programs.

To be considered for this role you will have:

  • Tertiary qualifications in Mental Health Nursing
  • A minimum of 5 years experience in a mental health setting, either in a clinical service role or supporting and coordinating a mental health service program
  • Comprehensive knowledge of mental health disorders and service provisions, in particular, primary care and health pathways
  • Demonstrated understanding of contemporary mental health treatments and a sound understanding of clinical governance and risk management principles
  • Strong communication and interpersonal skills with the ability to work in a collaborative team environment
  • Strong IT and systems skills with an ability to think critically and strategically, and support research, reporting, and data analyses
