We provide Aged Care, NDIS services, Meals on Wheels and Community Transport in Moree and Mungindi.

The Client Care Manager is a new position, underpinning our service delivery growth. The focus of the role is on managing our care services to meet the needs of older people and people with disability. This will include working with the Executive Officer and other Managers to establish and manage our new Home Care Package service and performing on-going Support Coordination for NDIS Participants.

The role may suit an RN who is looking to build on their clinical experience and move into a management role.

Key responsibilities

  • Managing our whole-of-service response to the care needs of people with Home Care Packages
  • Managing clinical care services and governance
  • Coordinator of Supports for NDIS Participants.

This will include:

  • Overseeing the operations of the HCP program in accordance with best practice clinical care and governance
  • Performing the role of Coordination of Supports for NDIS Participants
  • Developing and implementing policy and procedures to meet the Aged Care and NDIS compliance standards
  • Ensuring cost effective allocation of staff while also meeting Awards and HR principles, and taking account of the requirements of individual funding packages and government funding agreements
  • In conjunction with the Operations Manager, ensuring staff are trained to meet Moree Care’s service delivery obligations, values and client requirements
  • Providing day to day supervision and support to staff delivering care services.

Competencies required

The essential experience and competencies required for this role include demonstrated:

  1. Knowledge and experience working with older people and/or people with a disability and/or socially disadvantaged community members
  2. Knowledge and experience with program planning, implementation and evaluation
  3. Ability to provide leadership to a work team while managing competing demands and your own workload
  4. Strong interpersonal skills and a commitment to person-centred service delivery
  5. A high degree of initiative and the ability to negotiate and resolve issues.

Tertiary qualifications in a relevant health or social science discipline would be highly regarded - e.g. Registered Nurse or Allied Health qualifications.
