The Customer Service Liaison Officer will enable and facilitate a customer focussed service entry point for provision of services in the Commonwealth Home Support Program. The position will establish service relationships for customers that provide information, advice and support that facilitates effective and efficient service response. The position will also be responsible for coordinating, developing and reviewing client information which underpins goal directed care plans.

The ideal candidate will have:

  • Tertiary qualification in community, health services, case management or related service sector, or a certificate level qualification with significant and relevant experience
  • Demonstrated ability to develop and implement solutions to complex or sensitive client issues
  • Strong skills in the development of individualised care plans inclusive of personal care, to meet client identified goals in an active service approach
  • An understanding of client access pathways and the (CHSP) assessment mechanisms and their interface with service delivery activities and outcomes
  • Current Drivers Licence and Working with Children Check
