Lighthouse provides therapeutic care programs to children and young people impacted by long-term neglect, abuse and homelessness. Our focus is to create caring communities where young people can feel safe, form meaningful relationships and begin to heal. All Lighthouse programs, are underpinned by an evidence-based therapeutic model of care, called the Lighthouse Model of Care (LMC). The LMC integrates trauma-informed practice, attachment theory, and psychoanalytic principles of emotional development to create a holistic therapeutic treatment environment.
This role’s purpose is to provide Therapeutic Case Management to a low case load of children and young people, currently 2 young people in out of home care, who require a 24/7 therapeutic staffing model. Utilising a trauma informed practice to create a holistic therapeutic treatment environment; the primary duties will involve case management to a low case load of children and young people and supporting the Therapeutic House Supervisor to deliver individualised and community therapeutic interventions. You will support the culture in the homes by leading Operational Team Meeting for Therapeutic Carers, as well as role-modelling Lighthouse’s Model of Care by participating in Clinical spaces. The Therapeutic Case Manager works within a multi-disciplinary team, including Child Protection, the Lighthouse Foundation Clinicians and the Secure Base Team Leader. You will work predominantly from the lighthouse Foundation Youth Resource Centre, with some predictable visits scheduled to the homes.
