The role
We are seeking a full time Team Leader to join our Mental Health team for our Housing Accommodation and Support Program (HASP) with the position based in Salisbury on a contract basis until 30 June 2020 with possible extension.
HASP is delivered in partnership with clinical mental health services and a housing provider to provide intensive Psycho-social support which is delivered via home-based outreach services. The overall aim of the program is to assist consumers to live successfully in their communities by enhancing their life skills, resilience, and self-management of mental health issues. Support services are delivered through a Recovery Framework. You will be required to have skills in the delivery of Psycho-social Rehabilitation Support Services, supervision of staff and program implementation. Experience in working with the NDIS will be advantageous.
Key Responsibilities
Coordinate the daily operations of the program.
Coordination of the delivery of Individual plans and case plans for clients, including transition to the NDIS
Maintain positive relationships with other stakeholders (i.e. accommodation services, parents and families/carers).
Supervise and provide leadership to support workers.
The identification and development of community resources that can be used by consumers.
Ensure service provision in line with individual client plans, funding body standards and agreements, LWB policies and procedures and the LWB Way.
Skills & Experience
Tertiary degree in the Health a/o Human Services.
Demonstrated experience in the mental health or other health and welfare related fields with significant co-ordination experience.
Demonstrated knowledge of the recovery framework and its application in service delivery
Demonstrated knowledge of NDIS and experience in supporting clients in accessing the scheme
Sound team leadership experience including strategic analytical thinking, planning and delivering.
High level communication, influencing, liaison and negotiation skills including sound written and verbal skills. Ability to develop positive relationships with a broad range of internal and external stakeholders.
Strong written and computer skills for the creation of plans, programs, reports, correspondence and other documents.
Established problem solving and conflict management skills.
Commitment to the values of dignity, respect, inclusion empowerment, confidentiality and strength-based practice.
Current Driver Licence.
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
