About the Role

The Support Coordinator, Disability is to assist NDIS participants in the support coordination and implementation of all supports in their funded plan. The NDIA funds 3 levels of Support Coordination including;  

  • Level 1 - Support Connection
  • Level 2 - Coordination of Supports
  • Level 3 - Specialist Support Coordination

Key Responsibilities

  • Provide Level 1 or 2 Support Coordination 
  • Support the customer to implement their NDIS plan
  • Monitor plan outcomes and expenditure to ensure the plan remains in budget
  • Strengthen and enhance the customers abilities to self-direct their own supports and participate in the community 
  • Ensure mainstream services meet the obligations to the customer (Housing, education, justice, health) 
  • Clear and accurate record keeping of information
  • Build or support the capacity of teams to develop relationships and partnerships in local communities 

Skills & Experience

  • Certificate III, IV or Diploma in Disability or relevant Tertiary qualifications 
  • Demonstrated experience in working with people with a disability 
  • Experience in facilitating personal planning processes
  • Excellent verbal communication skills and a demonstrated ability to engage with clients and their families
  • Strong written and computer skills for the creation of plans, programs, reports, correspondence and other documents 

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
