The Swan View transitional accommodation service provides support 24 hours a day in a small residential setting for stays usually between 12 to 18 weeks, for up to three individuals leaving hospital. This service is designed and funded to complement, not replace, supports available from other systems such as the NDIS, health, education, and community-based services. This service is funded by the Western Australian Department of Health.
The Senior Support Worker is responsible for supervising the support of up to 3 adults who are experiencing complex mental health challenges.
The senior support worker focuses on planning for the individuals and liaison with external stakeholders to facilitate a successful transition into the service and preparing readiness for service exit. The senior support worker will promote positive mental health by supporting those in our service through involvement in delivering safe services, encouraging recovery and individual creativity, and embracing a holistic approach to wellness. Reporting to the Program Manager of Mental Health, this role has oversight of the direct care services to clients, day to day running of the service, provide supervision and guidance of the staff team, and monitoring and reporting on the program's performance and effectiveness.
This role can either be part time (4 weekdays) or full time, Monday to Friday until 31st August 2024. The hourly rate is a Level 3 SCHADS award (starting at $34.04).
Successful candidates will be required to clear probity checks including National Criminal History Record Check and NDIS Worker Screening check.
