About the role
Based in our Leederville office and supporting the wider WA Disability, Aged Care and Mental Health Services this role's key responsibilities is to finalise the initial entry process for clients referred to LWB and provide clients and their families clear information and advice about Life Without Barriers DACMH services. Working closely with our Client Intake & Support Officer, the successful candidate will engage with clients during the onboarding process to support service agreements that reflect their needs and preferences. Experience with NDIS and/or Mental Health intake, along with a passion for providing clients with the best possible onboarding experience is essential.
Key Responsibilities
- Participate in Panel referrals
- Initial meet and greet with Client/Family to complete the Engagement Form initiated by the Intake Officer and to gather any further additional information
- Complete and implement internal transition plan
- Understand and be aware of LWB SIL vacancies, ICLS availability and Aged Care service availability
- Building relationships with Leasing Agencies and resourcing of suitable properties for clients including viewings and lodging applications for properties and attendance at stakeholder meetings to ensure a smooth transition for the client
- Develop client profile in client database (CIRTS) ensuring all documentation is uploaded to appropriate areas
- Where applicable manage the client matching tool
- Support and promote a culture where Client First, Safety Always is an integral part of your role and our service delivery
- Adhere to organisational and legislative Health, Safety and Environment requirements.
Skills & Experience
- Demonstrated understanding of social welfare sector including Disability, Aged Care or Mental Health Services in a client facing or administrative position
- Understanding of person-centred thinking, the principles of the NDIS and the National Disability Service Standards or Mental Health services
- Knowledge of relevant community networks and mainstream disability services
- High level organisation and time management skills, which includes the ability to prioritise workload to meet changing demands.
- Demonstrated skills in using client information data bases and recording systems.
- High level written and oral communication, interpersonal, liaison, negotiation and customer service skills.
- Ability to manage sensitive information and maintain a high degree of confidentiality.
- Strong customer service and communication (both written and verbal) skills
- An ability to build relationships and work effectively with a range of stakeholders
- Experience working in a dynamic, high volume work environment
- Current WA Drivers Licence
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check.
