About the role

This is a rewarding opportunity for you to support our clients in the community and help them to engage and develop interests and increase access to the local community.

We have an initial 6 hrs per fortnight available, these hours may increase over time. Shifts days and times vary to suit our client’s requirements.

Responsibilities:  

• Promote and assist Clients to achieve goals identified in Person Centred Plans
• Assist Clients to develop independence and community networks 
• Support our client’s interests 
• Establish a rapport and collaborate effectively with clients and their carer to achieve positive outcomes
• Maintenance of progress notes and record keeping

Skills & Experience

• Certificate III in Individual Support (Disability) or working towards
• Ability to demonstrate interpersonal skills with clients, families and stakeholders
• Experience working with clients with challenging behaviours
• Demonstrate confidentiality and professional boundaries
• A current First Aid Certificate
• A current unrestricted Driver Licence  

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
